How does the Hypertension Care Programme (HCP) work?

Hypertension, or high blood pressure, is a common condition in which the pressure in your blood vessels is too high (140/90 mmHg or higher). Excessive pressure can harden arteries, decreasing the flow of blood and oxygen to the heart. Hypertension is a major risk factor for cardiovascular disease, including heart attack and stroke. It can also lead to kidney disease, vision loss, and other health problems if left untreated*.

The HCP is a 6-month programme for all our PRUShield policyholders that consists of three GP consultations, including standard medications and necessary screening tests like electrocardiogram (ECG), smoking cessation support (where applicable) and lifestyle counselling.

How much does the Hypertension Care Programme cost?

Preventive Screening:
Lipid Test*, Blood Sugar Test*, Blood Pressure and Body Mass Index (BMI)^ Assessment
Payment : S$0.00 nett

Your blood pressure will be taken. If it exceeds the normal range, you will be asked to rest for 10 to 15 minutes before the blood pressure is taken again. If the blood pressure is still high, you may have hypertension and the staff may enrol you in the Hypertension Care Programme.

You will also take the blood sugar and lipid tests. If the results exceed the normal range, you may then decide to proceed with the Diabetes Care Programme or High Cholesterol Care Programme.

^ If your BMI exceeds the normal range, the nurse will refer you to a GP for further follow-up as appropriate.

The cost of HCP for Singapore citizens is not more than S$100.00 nett. Raffles Medical Group clinic will apply the relevant government subsidies (Healthier SG, Primary Care Network, etc.) before submitting the bill for MediSave assessment. Customers will only pay the balance after all subsidies and MediSave deductions have been made.

For Singapore Permanent Residents do check with the clinic for the relevant subsidies. Foreigners do not receive any government subsidies, but the cost will not exceed S$300.00 nett. Please refer to the FAQs below for more details.

If policyholder chooses to continue with Raffles Medical for treatment, Raffles Medical will offer 10% off medications.

  • Blood test - kidney assessment (including urine test)*
  • Lifestyle Counseling
  • Smoking Cessation support (where applicable)

FAQs

All PRUShield policyholders may book an appointment via PRUServices to undergo a blood sugar test, blood pressure test, lipid test, and BMI assessment at any of the 37 Raffles Medical clinics in Singapore at no cost. Walk-in appointments for this programme are not permitted. Fasting from midnight prior to the appointment is required to ensure more reliable test results. This service is only applicable to PRUShield policyholders who have not been diagnosed with hypertension.

Once your blood pressure is taken, and if it exceeds the normal range, you will be asked to rest for 10 to 15 minutes before the blood pressure is taken again. If the blood pressure is still high, you may have Hypertension, and the staff may offer you the (HCP).

The 6-month HCP includes three GP consultations with standard medications and a primary care network team to track and guide your progress. During the three consultation, an electrocardiogram (ECG), blood and urine tests will be provided. The test results will be reviewed by the GP.

Every consultation will include a GP review and a 2-month supply of standard medications. However, during the 6-month HCP, you will undergo smoking cessation and lifestyle counselling which can take place at any of the consultations. On the third GP consultation, you will be advised on future treatment options. A final 2-months' supply of standard medication will be provided to you, and this will conclude the 6-month HCP.

If you have requested for the other tests and the results exceed the normal range, you may be offered the following:

The HCP is offered to all PRUShield policyholders. PRUShield policyholders may submit an online appointment request for the blood pressure, blood sugar and BMI assessment at any of the 37 Raffles Medical Group clinics once every calendar year.

Policyholders who have been previously diagnosed with Hypertension/Diabetes and are undergoing treatment with a GP will not be eligible as they would already be on existing medication prescribed by their attending physician. The HCP and/ or DCP can only be utilised once and policyholders who have previously participated in DCP and/or HCP will not be eligible to re-enrol.

All medical information of policyholders obtained under the HCP are confidential and Raffles Medical Group will not share the policyholder’s medical records or test reports with Prudential Singapore.

If blood pressure / body mass index (BMI) / lipid test results exceed the normal range, you may decide to enroll for:

  • Easy claims process through MediSave applicable for Singapore Citizens and Permanent Residents only, subject to their annual limits
  • Dedicated patient care team to track and guide you throughout the care journey
  • Face-to-Face GP Consultations with access to an extensive Raffles Medical's network of 37 clinics islandwide
  • Receive lifestyle counselling and smoking cessation support (where applicable) during the course of the consultations
  • Comprehensive tests and screenings, inclusive of Electrocardiogram (ECG) and GP consultations
  • Standard Medication will be included during the course of your care programme

In addition, PRUShield policyholders will be entitled to a 10% discount on non-standard medications.

Fasting from midnight before the appointment date is mandatory, to yield more reliable results. If you are taking regular medications, please continue to take them as prescribed, but only with water.

The first GP consultation will be arranged once your ECG and blood test results are ready. Thereafter, at the end of each GP consultation, the clinic nurse will assist you to book your next appointment.

You will receive two SMS* which will be sent to you upon confirmation of your appointment at your preferred Raffles Medical Clinic.

*SMS will be sent to you seven days before and two days before your appointment date with instructions to fast.

This service is only available for all eligible PRUShield including PRUPanel Connect policyholders. Please check for your eligibility through PRUServices.

Walk-in appointments are not available for Hypertension Care Programme. To help manage waiting times for our PRUShield policyholders, appointments must be booked online via PRUServices. After logging in to PRUServices, please select your PRUShield policy and go to More Services > Chronic Care Preventive Tests.

Yes, you can use MediSave for the HCP. MediSave benefits are applicable only for Singapore citizens and Permanent Residents, subject to MediSave balance and withdrawal limits, where co-payment is required. The clinic staff at Raffles Medical Group (RMG) will assist you with the filing of the MediSave claim.

Please note that Singapore citizens are also eligible for additional subsidy for the HCP. Singapore citizens enrolled under Healthier SG signed up at the RMG clinic are also eligible for additional subsidy for this programme.

Singapore citizens and Permanent Residents who are not using MediSave as well as Foreigners will need to make full payment after the consultation.
