How does Hypertension Care Programme (HCP) work?

Hypertension, or high blood pressure, is a common condition in which the force of blood against the walls of the arteries is consistently too high. It puts strain on the arteries and cause damage to the blood vessels and organs over time. Hypertension is a major risk factor for cardiovascular disease, including heart attack and stroke. It can also lead to kidney disease, vision loss, and other health problems if left untreated.

The HCP is a 6-month programme that consists of a pre-consultation and three GP consultations during the duration of the programme. The programme will include standard medications and also the necessary screening tests like ECG, smoking cessation (where applicable) and lifestyle counselling.

How much does the Hypertension Care Programme cost?

 

Blood Pressure*, Blood Sugar Test and Body Mass Index (BMI)* Assessment
Payment : S$0.00 nett

Your blood pressure will be taken. If it exceeds the normal range, you will be asked to rest for 10 to 15 minutes before the blood pressure is taken again. If the blood pressure is still high, you may have Hypertension and the staff may offer you the Hypertension Care Programme.

*If your blood sugar test result appears elevated, you will be referred by a nurse to a GP for further follow-up as appropriate and you may then decide to proceed with the Diabetes Care Programme.

The cost of HCP for Singapore citizens is S$100.00 nett, with a co-payment of S$20.00 and the remaining $S$80.00 can be deducted from MediSave, subject to balance and withdrawal limits. Singapore citizens enrolled under Healthier SG signed up at the RMG clinic are also eligible for additional subsidy for this programme.

As Singapore permanent residents and foreigners do not receive any government subsidies, the cost will be S$150.00 nett. Co-payment is applicable for policyholders using MediSave funding, subject to MediSave approval. MediSave benefits are applicable only for Singapore citizens and Permanent Residents. FAQs

How does the Hypertension Care Programme (HCP) work?

All PRUShield policyholders can book an appointment here to request for Blood Pressure Test, Blood Sugar Test and BMI check at any of the 37 Raffles Medical clinics in Singapore at no cost. It is mandatory to fast from midnight before the appointment date to yield more reliable results.

Once your blood pressure is taken, and if it exceeds the normal range, you will be asked to rest for 10 to 15 minutes before the blood pressure is taken again. If the blood pressure is still high, you may have Hypertension and the staff may offer you the Hypertension Care Programme.

The Blood Sugar Test involves a pinprick on the finger to extract a small drop of blood. This is used to measure your blood sugar level. If your blood sugar test result appears elevated, you will be referred by a nurse to a GP for further follow-up as appropriate and you may then decide to proceed with the Diabetes Care Programme. If your BMI exceeds the normal range, the nurse will refer you to a GP for further follow-up as appropriate.

The 6-month HCP includes a pre-consultation and three GP consultations with standard medications and a primary care network team to track and guide your progress. During the pre-consultation, an electrocardiogram (ECG) and blood tests will be provided before you attend your first consultation session. The test results will be reviewed by the GP during the first consultation. Every consultation will include a GP review and a 2-month supply of standard medications. However, during the 6-month HCP, you will undergo smoking cessation and lifestyle counselling which can take place at any of the consultations. On the third GP consultation, you will be advised on future treatment options. A final 2-months' supply of standard medication will be provided to you, and this will conclude the 6-month HCP.

Who is eligible for the Hypertension Care Programme (HCP)?

The HCP is offered to all PRUShield policyholders. PRUShield policyholders may submit an online appointment request for the Blood Pressure, Blood Sugar and BMI at any of the 37 Raffles Medical Group clinics once every calendar year.

Policyholders who have been previously diagnosed with Hypertension/Diabetes and are undergoing treatment with a GP will not be eligible as they would already be on existing medication prescribed by their attending physician. The HCP and/ or DCP can only be utilised once and policyholders who have previously participated in DCP and/or HCP will not be eligible to re-enrol.

Will Prudential have access to any of my medical reports obtained under the Hypertension Care Programme (HCP)?

All medical information of policyholders obtained under the Hypertension Care Programme are confidential and Raffles Medical Group will not share the policyholder’s medical records or test reports with Prudential Singapore.

What happens if my blood sugar and/ or body mass index (BMI) readings are high but my blood sugar test is normal?

If your blood sugar test result appears elevated, you will be referred by a nurse to a GP for further follow-up as appropriate and you may then decide to proceed with the Diabetes Care Programme. If your BMI exceeds the normal range, the nurse will refer you to a GP for further follow-up as appropriate.

Do I need to fast prior to my appointment?

Fasting from midnight before the appointment date is mandatory, to yield more reliable results. If you are taking regular medications, please continue to take them as prescribed, but only with water.

How do I schedule the face-to-face GP consultations for the Hypertension Care Programme (HCP)?

The first GP consultation will be arranged once your ECG and blood test results are ready. Thereafter, at the end of each GP consultation, the clinic nurse will assist you to book your next appointment.

What happens if I miss my medication?

Will I receive any confirmation SMS from Raffles Medical upon submission of appointment booking?

You will receive two SMS* which will be sent to you upon confirmation of your appointment at your preferred Raffles Medical Clinic.
*SMS will be sent to you two calendar weeks before and two days before your appointment date with instructions to fast.

Can I extend this service to my family members and friends?

This service is only available for all eligible PRUShield including PRUPanel Connect policyholders. Please check for your eligibility using this link: www.prudential.com.sg/ppc-validate

Can I walk into any of the Raffles Medical clinics for the Hypertension Care Programme (HCP)?

Unfortunately, walk-in appointments are not available for Hypertension Care Programme. To better manage waiting times for our policyholders, an online appointment must be made via the Prudential PRUPanel Connect website

Can I use MediSave for the Hypertension Care Programme (HCP)?

Yes, you can use MediSave for the HCP. MediSave benefits are applicable only for Singapore citizens and Permanent Residents, subject to MediSave approval, where co-payment is required. The clinic staff at Raffles Medical will assist you with the filing of the MediSave claim.

Please note that Singapore citizens are also eligible for additional subsidy for the HCP. Singapore citizens enrolled under Healthier SG and who have signed up at RMG clinics are also eligible for additional subsidies. Singapore citizens and Permanent Residents who are not using MediSave as well as Foreigners will need to make full payment during the pre-consultation.
